Special Disclosures Required by Law

Filing a Privacy Complaint
If you believe your privacy rights have been violated, you may file a complaint directly with our practice staff or with the Secretary of the U.S. Department of Health and Human Services (HHS). You will not face penalty or retaliation for filing a privacy complaint.
