Your Right to a Good Faith Estimate (No Surprises Act)

Under federal law, healthcare providers must furnish patients who do not have insurance, choose self-pay, or use out-of-network benefits with a Good Faith Estimate (GFE) detailing expected costs before services are rendered.

Delivery Timeline

Provided in writing within 1–3 business days after scheduling or upon pre-service request.

Full Statutory Notice of Right to Receive a Good Faith Estimate

  • Request Rights: You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare items or services upon request or when scheduling care.
  • Provider Obligations: Your estimate must include expected charges for primary therapy sessions as well as any related diagnostic, intake, or administrative fees.
  • Documentation & Records: Always save a written copy or picture of your Good Faith Estimate and your final billing invoices for comparison.
  • Dispute Process: Initiating a dispute will not impact the quality or availability of your treatment. If you receive a bill that is at least $400 over your estimate, you may start the Patient-Provider Dispute Resolution (PPDR) process within 120 calendar days of receiving the invoice.
